POSITION SUMMARY:
Reporting to the Director of Clinical Services, the Licensed Practical Nurse is committed to providing the highest quality of care to patients in their homes.
EDUCATION AND QUALIFICATIONS:
· Currently licensed in the State of Florida as a Licensed Practical Nurse (LPN)
· One year of nursing experience in skilled home health required
· Current valid IV certification is required
· Must maintain current CPR certification
· Valid driver's license and automobile insurance in the State of Florida; must have reliable transportation to be able to meet visit schedule
ESSENTIAL JOB RESPONSIBILITIES:
· Possesses the ability to prioritize patient needs on a daily basis
· Participates in the team responsibility of providing quality care
· Exhibits professionalism and responsibility within and outside the organization
· Participates in rotation of back-up and on call requirement when necessary
· Is financially conscientious in providing cost-effective nursing care
· Effectively communicates with the Home Health RN when needed in discussing matters related to nursing processes and services
· Collaborates with the RN in reviewing and developing Care Plans
· Actively participates in weekly Interdisciplinary Group (IDG) Meetings
· Maintains a good working relationship with team members and other disciplines in with a common goal to provide exceptional patient care
· Attends to patient and family complaints/concerns while communicating issues clearly with RN.
· Maintains a pro-active approach, assuring problem resolution is performed in an effective, timely manner. Uses persuasiveness and good judgment to formulate appropriate win-win outcomes when possible
· Possess the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with Gulfside Home Health standards
· Ensures all patient care and documentation of care is compliant with Gulfside Home Health standards, Florida State and Federal Regulations
· Ensures professional standards of Home Health nursing practices are maintained
SKILLS AND COMPETENCIES:
· Maintains up to date skill sets as required to care for patients.
· Possesses ability to multi-task; exhibits adaptability and flexibility in meeting Gulfside Home Health Care objectives.
· Ensures compliance with all patient care and documentation standards and regulations.
· Assists in the implementation and maintenance of the policies and procedures of the nursing department; adheres to and assists in the evaluation of the same.
· Assists in maintaining federal and state regulatory compliance.
· Proficient in Electronic Medical Records, emails and basic computer knowledge.
